Resident identifiers: #62, #20. Facility census: 62. Findings included: a) Resident #62 Record review indicated Resident #62, [AGE] years of age, was administered 13-valent pneumococcal conjugate vaccine (PCV13) vaccine on 08/17/16. No documentation was found to indicate the resident either received the 23-valent pneumococcal [MEDICATION NAME] vaccine (PPSV23) or did not receive the vaccine due to medical contraindications, previous vaccination, or refusal. During an interview on 02/12/20 at 10:05 AM, Infection Control Nurse (ICN) #66 verified there was no evidence to indicate the facility made an attempt to determine if the Resident had been offered or administered the PPSV23 vaccine. ICN #66 stated the facility had no process or procedure in place for monitoring the provision of both vaccines (PPSV23 and PCV13 vaccines) and said, I didn't know there were two vaccines and they both had to be given. Review of the facility's Pneumococcal Vaccine (Series) policy implemented on 11/27/17, stated the type of pneumococcal vaccine will be offered to Residents in accordance with current CDC guidelines and recommendations. The policy further stated: Each resident will be assessed for pneumococcal immunization upon admission. Self-report of immunization shall be accepted.
