In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #12's 30-day minimum data set (MDS) assessment for section M, Skin conditions revealed the dimensions of unhealed stage 3 or 4 pressure ulcers or eschar was entered incorrectly. Resident identifier: #12. Facility census: 40. Findings include: a) Resident #12 A review of the MDS, assessment reference date 03/22/17, showed the resident had one (1) stage two (2) pressure ulcer. Further review of the MDS, revealed measurements for length and width for a stage three (3) or four (4) pressure ulcer. An interview with Assistant Director of Nursing (ADON) #42 on 06/07/17 at 1:38 pm, advised she had entered the measures incorrectly in this section.
