In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This was true for one (1) of one (1) medication refrigerators and has the potential to affect all residents residing in the facility. Facility census: 53. Findings included: a) On 07/09/19 at 3:00 PM, a review of the medication room with Licensed Practical Nurse (LPN) #76, revealed the medication refrigerator temperature was not documented daily. The Refrigerator Temperature Log for (MONTH) 2019, was blank on 07/02/19 and 07/04/19. During this observation, LPN #76 confirmed the temperature log for the medication refrigerator was incomplete. LPN #76 reported the facility policy is the 3-11 nurse checks the temperature and documents the findings. 2020-09-01
