In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This failed practice had the potential to affect a limited number of residents residing at the facility. Resident identifiers: #139 and #182. Facility census: 182. Findings included: a) Resident #139 Review of Resident #139's medical record found an order for [REDACTED]. Order effective 08/15/19. Review of the August, (MONTH) and (MONTH) 2019, Medication Administration Record [REDACTED]. Review of the monthly medication regimen reviews found the consultant pharmacist completed a medication review on 08/24/19, 09/09/19 and 10/03/19 with no recognition the [MEDICATION NAME] was being administered without the required heart rate and blood pressure obtained prior to the administration of the medication. Interview with the DON on 10/10/19 at 10:00 am, confirmed the resident's blood pressure and heart rate was not taken and documented prior to the administration of the [MEDICATION NAME] and the consultant pharmacist had not recognized the medication was being administered without the required blood pressure and heart rate. b) Resident #182 Review of Resident #182's medical record found an order for [REDACTED]. Order effective 06/17/19. Review of the June, July, and (MONTH) 2019, Medication Administration Record [REDACTED]. Review of the monthly medication regimen reviews found the consultant pharmacist completed a medication review on 06/21/19, 07/17/19, and 08/22/19, with no recognition the [MEDICATION NAME] was being administered without the required heart rate obtained prior to the administration of the medication. Interview with the DON on 10/10/19 at 10:00 am, confirmed the resident's heart rate was not taken and documented prior to the administration of the [MEDICATION NAME] and the consultant pharmacist had not recognized the medication was being administered without the required heart rate. 2020-09-01
