In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This was true for one (1) of three (3) residents reviewed for hospital transfers. Resident identifier: #95. Facility census 182. Findings include: a) Resident #95 During a medical record review, on 10/08/19, it was discovered that Resident #95 was transferred to a local hospital on [DATE] at 7:25 PM. There was no evidence the resident or the residents representative received a copy of the bed hold policy at the time of transfer. In addition there was no documentation in the medical record of contacting the resident / resident representative regarding the bed hold policy. During a record review on 10/08/19 at 1:47 PM, a copy of the bed hold notice could not be located on Resident #95's chart on the unit or in the thinned medical record. On 10/08/19 at 3:57 PM, the chart was given to the Director of Nursing (DON) for review to see if the bed hold notice could be located. The DON was unable to locate the bed hold notice. During an interview with the DON on 10/08/19 at 4:11 PM, the DON stated that she could not find the transfer form with the stamp that indicated that the notice of bed hold policy was provided upon discharge. The findings were discussed with the DON and Administrator on 10/09/19 at 8:22 AM and no further information was provided. 2020-09-01
