Findings include: a) On 07/30/09 at 3:55 p.m., a nursing assistant (Employee #12) was observed holding a pitcher over the ice chest when adding ice. This pitcher had been in a resident's room. This practice created a potential for contamination of the ice in the ice chest. Employee #52, another nursing assistant, intervened so no other resident was affected.
