In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This was evident for one (1) of the two (2) kitchens and had the potential to affect all residents on the 500 Hall who receive nourishment from that kitchen. Facility census: 122. Findings include: a) During the initial tour on 07/20/09, an attempt to discard a used paper towel revealed the step-on trash can at the employee handwashing station in Kitchen #2 was not functioning. A dietary staff member (Employee #180) directed the surveyor to throw her paper towel onto a tray of food that she was going to discard. On 07/21/09 at 11:30 a.m., observation of Kitchen #2 revealed no trash receptacle at the employee handwashing station. Further observation of Kitchen #2 found a large black, round plastic trash can with a fitted lid in the dishwashing area. There were no other trash receptacles in the kitchen. On 07/22/09 at 4:00 p.m., a repeat observation of Kitchen #2 again found no trash receptacle at the employee hand-washing station. This surveyor reported the observation to Employee #12, and she explained that the step-on trash can broke yesterday. She threw her paper towel into the large black, round plastic trash can with a fitted lid that was housed in the dishwashing area. This surveyor did the same but could not avoid touching the trash can with her hand as she disposed of a used paper towel. . 2014-12-01
