In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The call bells for room #108 did not function when tested by the surveyor. This was evident for one (1) of fourteen (14) sampled residents whose call bells were evaluated for functioning. Facility census: 112. Findings include: a) The surveyor evaluated the operation of nurse call bells on the afternoon of 10/06/09 for the residents in the Phase I sample. This performance testing discovered the call bells did not activate for the resident in room #108. The surveyor requested the nursing assistant (Employee #62) who was in the hall way to check the call light with her. The light was found to not operate for the resident in either bed in the room. . 2015-02-01
