In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The residents failed to have a detailed summary of clinical data analysis of findings under the supporting documentation of each triggered CAA. The findings included: The facility admitted Resident #6 with [DIAGNOSES REDACTED]. Based on record review 3-4-15, Resident #6 did not have a detailed description of the supporting documentation of which CAA has been triggered, and documentation of assessment information in support of clinical decision making relevant to the CAAs. The facility admitted Resident #2 with [DIAGNOSES REDACTED]. On 3/4/15 at 9:10 AM, review of the Minimum Data Set (MDS) CAAs (Care Area Assessments) revealed indicator for each of the Care Areas were checked in the appropriate boxes. Review of the Analysis of Findings revealed there was no documentation of summary information regarding the indicators for the triggered areas necessary for clinical decision making. The facility admitted Resident #4 with [DIAGNOSES REDACTED]. On 3/4/15 at 9:45 AM, review of the 3/1/15 Annual Minimum Data Set (MDS) CAAs (Care Area Assessments) revealed indicators for each of the Care Areas were checked in the appropriate boxes. Review of the Analysis of Findings revealed there was no documentation of summary information regarding the indicators for the triggered areas necessary for clinical decision making. The facility admitted Resident #8 with [DIAGNOSES REDACTED]. On 3/4/15 at 11:18 AM, review of the 10/1/14 Annual Minimum Data Set (MDS) CAAs (Care Area Assessments) revealed indicators for each of the Care Areas were checked in the appropriate boxes. Review of the Analysis of Findings revealed there was no documentation of summary information regarding the indicators for the triggered areas necessary for clinical decision making. The facility admitted Resident #9 with [DIAGNOSES REDACTED]. On 3/4/15 at 10:58 AM, review of the 1/14/15 Significant Change in Status Minimum Data Set (MDS) CAAs (Care Area Assessments) revealed indicators for each of the Care Areas were checked in the appropriate boxes. Review of the Analysis of Findings revealed there was no documentation of summary information regarding the indicators for the triggered areas necessary for clinical decision making. During an interview on 3/4/15 at 4:30 PM, the MDS Coordinator confirmed there was no summary of the indicators for the triggered CAAs for Residents #2, #4, #6, #8, or #9. The MDS Coordinator stated they implemented a new computer system in (MONTH) and that no comprehensive assessments completed since then had a synopsis of information for clinical decision making. The MDS Coordinator also stated that the training they received on the new computer system did not include writing an analysis of the findings after checking the boxes as appropriate for the individual residents. Review of Appendix C of CMS's (Center for Medicare and Medicaid Services) RAI (Resident Assessment Instrument) Version 3.0 Manual dated (MONTH) 2010 provided by the facility revealed Step 6: Analyze the findings in the context of their relationship to the care area. This should include a review of indicators and supporting documentation, including symptoms and causal and contributing factors, related to this care area. Draw conclusions about the causal/contributing factors and effect(s) on the functional ability of the resident, and document this information in the Analysis of Findings Section. 2018-10-01
