In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The finding included: Resident #2 was admitted to the facility on [DATE] with [DIAGNOSES REDACTED]. Review of complaint records indicated Resident #2 was involved in five incidents of resident-to-resident altercations between November 2018 and September 2019 in which Resident #2 was the physical aggressor. On 10/16 at approximately 3:30 PM, reviewed of Resident #2 medical book revealed there was no care plan for Resident #2's behaviors. Also there was no documentation on Resident #2 being picked on by other residents. During an interview on 10/16/2019 at approximately 2:20 PM, the Director of Nursing (DON) stated that Resident #2 was being picked on by others residents which caused Resident #2 to get upset and act out by having altercations with other residents. The DON stated that Resident #2 was on informal 15 minutes checks and we have him/her stay around the nursing station because we can keep him/her in our sights. Also, the DON stated that Resident #2 does not have a care plan to address his/her altercations with other residents and there was no documentation for the 15 minutes checks because the DON decided that Resident #2 was being picked on, which makes him agitated. On 10/16/19 at approximately 3:30 PM, review of the facility policy entitled Adult or Elder Abuse, Mistreatment and Neglect revealed: It is facility policy to protect residents from abuse, neglect, mistreatment or exploitation from anyone, including staff members, students, volunteers, other residents, consultants' staff or other agencies serving the resident, family members, legal guardians, friends, or visitors. 2020-09-01
