In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Review of the most recent annual Minimal Data Set (MDS) assessment dated [DATE] assessed resident # 41 as currently being a hospice resident. The record revealed the resident was admitted to hospice on 10/21/11. Review of the record revealed the resident had two (2) sets of care plans, one that was done by the facility that was initiate in August 2011 and the other that was done by hospice and initiated 4/17/2012. The two (2) care plans included some of the same problems but each had different goals and interventions. The facility's care plan did not address the psychosocial and spiritual needs for the resident. The hospice care plan addressed the spiritual needs of the resident but not the care needs. Interview with MDS Coordinator 4/18/12 at 1:29 p.m. confirmed that the care had not been updated to coordinate the care of the resident since he/she was admitted to hospice care. 2016-06-01
