In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: The State Agency generated report of Missing/Late MDS assessments was reviewed with MDS coordinator BB on 1/24/12 at 3 p.m. and on 1/25/12 at 1:15 p.m. One of the seventeen residents listed in the report did not have his/her comprehensive assessment completed timely as required. One resident had an annual comprehensive MDS completed by staff on 4/24/10. The staff should have completed the next required annual comprehensive assessment in April 2011 but, they did not do it. The facility did not complete a comprehensive assessment of the resident until 5/23/11, when there had been a significant change in his/her condition. 2016-06-01
