In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: The State Agency generated report of the facility's Missing/Late MDS assessments was reviewed with MDS coordinator BB on 1/24/12 at 3 p.m. and on 1/25/12 at 1:15 p.m. The facility failed to complete two residents' required MDS for their comprehensive admission assessments. 1. One resident was admitted to the facility on [DATE]. Staff should have completed his/her MDS within 14 days of the admission (9/13/11), however, staff did not complete it. 2. The admission MDS was due to have been completed for one resident prior to his/her death in the facility on 10/29/10 but, staff did not do it.	2016-06-01
