In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: During observations of lunch having been served in the main dining room on 1/23/12 at 12:40 p.m., on 1/24/2 at 12:45 p.m. and 1/25/12 at 12:45 p.m., five (5 ) to eight ( 8) residents were observed eating with plastic spoons. In an interview on 1/26/12 at 9:55 a.m., the Dietary Manager stated that the kitchen staff had just told him that they had run out of metal spoons. After surveyor inquiry at that time, the Dietary Manager stated that he was going to buy more eating utensils. 2016-06-01
