In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Record review revealed a Minimum Data Set (MDS) for resident #65 dated 01/30/12 which assessed the resident as receiving antipsychotic medications. Review of physician's orders [REDACTED]. The Care Area Assessment (CAA) dated 02/13/2012 indicates that [MEDICAL CONDITION] drug use was triggered and would be addressed in the Care Plan. Review of the care plan dated 02/21/12 revealed no care plan for [MEDICAL CONDITION] medications. Interview with MDS Coordinator CC conducted 02/29/12 at 10:50am. revealed, that after reviewing the CAA, a care plan should have been developed for [MEDICAL CONDITION] drug use. Interview with Nursing Supervisor AA conducted 02/29/2012 at 11:00am revealed the resident is receiving [MEDICAL CONDITION] medications and should have been included in the care plan. 2016-06-01
