Findings include: Resident (R) #1 had a care plan since 3/8/17 for being weak most days and needing help and reminders not to fall. The care plan included an intervention for Certified Nursing Assistants (CNA's) to use a Hoyer lift and at least two people to assist with all transfers. There was an additional care plan for falls, included in the electronic portion of the clinical record, dated 7/17/18, that also included an intervention for staff to use a Hoyer lift with the assistance of two staff members for all transfers. However, on 8/11/18, R#1 was transferred from the bed to a geri-chair and from the geri-chair to the bed without the use of a Hoyer lift as care planned. During an interview on 8/27/18 at 2:50 p.m. CNA AA stated that she transferred the resident out of bed to the geri-chair and later transferred her back to bed on 8/11/18. She confirmed that a Hoyer lift was not used to assist with the transfers. During an interview on 8/28/18 at 12:15 p.m., CNA BB, who was also present when R#1 was assisted back to bed on 8/11/18, confirmed that a Hoyer lift was not used to assist with the transfer.
