These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

1128 RIVERSIDE HEALTH CARE CENTER 115375 5100 WEST ST NW COVINGTON GA 30014 2017-08-17 253 E 0 1 I6YR11 Based on observations, interview and record review, the facility failed to ensure the building was adequately clean and maintained in three of four resident units (100, 200, and 400 rooms) and in common areas. The census was 147 residents. Findings include: 1. Environment observations/tour a. 100 Hall Room 101B - on 8/16/17 at 9:49 a.m. and during the environmental tour, on 8/17/17 between 9:45 a.m. and 11:06 a.m., five red splatters or spills were observed low on the bedroom wall and several more on the floor adjacent to the bathroom. During the environmental tour with the Maintenance Director (MD), Maintenance Staff, and Director of Housekeeping (DH) on 8/17/17 between 9:45 a.m. to 11:06 a.m. the same splatters were observed. The MD stated housekeeping staff was responsible to clean the splatters off the walls and floors. Flooring in the hallway - on 8/16/17 at 10:08 a.m. and during the environmental tour, on 8/17/17 between 9:45 a.m. and 11:06 a.m., cracks were observed on all the linoleum floor tiles (linoleum) extending across the hallway at the start of the lower numbered section of the 100 hall. In addition, in the higher numbered sections, across from the oxygen storage room, seven tiles were cracked in a row down the hallway creating a slight edge between the higher and lower part of the tiles. Twelve more tiles were cracked length wise down the hallway outside rooms 109 through room 111. The MD was interviewed during the environmental tour on 8/17/17 between 9:45 a.m. to 11:06 a.m. and stated it was a foundation issue which affected the flooring. He stated there had been a large tree causing the flooring to buckle and tiles to crack on the 100 hall. The MD stated the tree was removed about six months ago, and he was still watching for additional settling. The MD stated the facility planned to build an addition onto the 100 hall and all the flooring in the hallway would be replaced at that time. The MD stated the facility was in the planning phase of this project and it had not yet been approved by government officials. The MD stated the linoleum tiles seen throughout the facility were no longer available due to the age of the linoleum tile, so an exact match for the tiles was not possible. Room 108 - on 8/16/17 at 10:15 a.m. and during the environmental tour, on 8/17/17 between 9:45 a.m. and 11:06 a.m., two discolored grey sections (approximately 1 foot long and several inches wide) of the white linoleum flooring were observed near the armoire. In addition, there were two beige spots/stains on the bed spread and a brown stain on a pillow approximately 3 inches in diameter. Room 114 - on 8/16/17 at 10:17 a.m. and during the environmental tour, on 8/17/17 between 9:45 a.m. and 11:06 a.m., a circular area of black residue approximately 1 inch in diameter was observed in the corner of the baseboard on the window side of the room. b. 200 Hall Flooring in the hallway - on 8/16/17 at 10:40 a.m. and during the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m., an area of eight cracked linoleum floor tiles, outside the women's bathroom, was observed. This resulted in a slight edge between the higher and lower parts of the tile. Main Dining Room - on 8/16/17 at 10:40 a.m. and during the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m., 10 tiles spanning the width of the hall, located at the entrance into the main dining room were stained pink (up to a maximum of approximately 3 inches in width) in contrast with the remaining white linoleum. The DH stated during the environmental tour, The pink is in the tile. It does not come out. The MD stated, We can replace it and showed the surveyor another area of replaced tiles that was not an exact match, but was a close match to the existing tiles. c. 400 Hall Room 406 - on 8/16/17 at 11:10 a.m. and during the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m., accumulated black residue approximately 1/2 inch wide was observed along the edge of floor under the air conditioner on the window side of the room. The DH was interviewed during the environmental tour and stated, We don't get the edges (of the flooring) with a deep clean. That would be a floor project. We strip the floor and clean the edges. We try to maintain it. Once a year we pull everything out (of each room to clean the floor). Room 407 - on 8/16/17 at 11:14 a.m. there was a basin under bed B with wet paper towels and a urine odor was noted. During the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m. an area of grey residue along the wall a section of approximately 2 feet up, about 1 foot from the floor was observed; a linoleum tile on the window side of the room was loose and partially pulled up; and an odor of urine permeated the room. Room 408 - on 8/16/17 at 11:05 a.m. and during the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m. accumulated black residue approximately 1/2 inch in width was observed along the edge of the floor where the bathroom and dresser were located. Room 410 - on 8/16/17 at 11:03 a.m. the resident in bed B was observed in bed with the sheet stained with a pink substance in 2 places, a 1 inch circle and a 2-3 inch long crescent-shaped area. During the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m., a grey marked area on the wall approximately 2 feet long and several inches wide, at approximately 3 feet up from the floor was observed. One tile was missing a corner section of 2 inches by 1 inch, by the air conditioner on the window side of room. Room 415 - on 8/15/17 at 9:38 a.m. the toilet seat was soiled/stained with a brown material. On 8/16/17 at 11:38 a.m. the bathroom was inspected and although the toilet seat was clean, there was a strong odor of both urine and bowel movement. During the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m. the urine odor was present; the MD stated the tile had been replaced recently in this room, but the resident had trouble urinating into the toilet and instead urinated on the linoleum floor. Area adjacent to dining room/emergency exit door - on 8/16/17 at 10:50 a.m., 8/16/17 at 11:48 a.m. and during the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m., the following were observed: -The corners by the exit door contained a layer of black grime 2 inches wide on the linoleum flooring. -There was a spider web on the inside door frame of the exit door. -There was black residue and grime caked in the metal plate/casing of the exit door. -There was a spider web behind the Emergency Exit sign which lead to the exterior door next to the dining room at the end of the 400 Hall. -There was a spider web on the bottom right corner of the glass in the exit door. -There was a continuous spider web along the hinge side of the exit door by the dining room at the end of the 400 Hall. -In the corner at the end of the hallway by the exit door there was debris on the floor, including seven small dead bugs. -Spider webs with gnats in the webs were observed in the corners above the exit door. The DH acknowledged during the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m. the area near the exit door and adjacent to the dining room was not clean and should be cleaned on a weekly basis. Dining Room 400 unit - on 8/16/17 at 10:50 a.m., on 8/16/17 at 11:48 a.m., and during the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m., the wall paper (section of approximately 2 feet in length) along the top of the large entrance area from the hallway into the dining room was bubbled and peeling away from the wall. During the environmental tour, the MD stated the plan had been to replace it on 8/14/17; however, they had been unable to do this due to the start of the survey on this date. 2. Interviews The family member of R#147 was interviewed on 8/14/17 at 4:32 p.m. and stated, The filters are dirty and full of lint while showing the surveyor an accumulation of lint in the air conditioning filter. During the environmental tour on 8/17/17 between 9:45 a.m. and 11:06 a.m. the DM stated the filter was full of lint and it would be cleaned. b. The Secure Unit (400 numbered rooms) Housekeeper was interviewed on 8/16/17 at 11:44 a.m. She stated she was the housekeeper in the 400 unit and she cleaned all the rooms daily. One housekeeper was designated for this unit. She stated she dusted, swept, mopped, and emptied garbage daily. In addition, she performed deep cleaning to one room per week. During the survey, 50 residents lived on the 400 unit. Maintenance Staff AA was interviewed on 8/16/17 at 11:34 a.m. He stated maintenance was responsible for doing room checks. The room checks were completed in June, but not in (MONTH) due to being short a maintenance staff member. He stated the checks had been reinstituted in August. He stated the MD had been going back and forth between two sister facilities due to a staff shortage and he too was going back and forth but spent most of his time at this facility. Maintenance Staff AA stated he was primarily located at this facility and reported there should be two full-time maintenance staff plus a third staff that did nothing but painting due to the large size and age of the building (census during the survey of 147 residents). 3. Record review a. Grievances Review of Grievance/Complaint Forms for a six-month period preceding the survey (2/14/17- 8/14/17) showed ongoing concerns with the provision of housekeeping and maintenance services: -On 2/14/17 a grievance was filed by a resident as follows: 1. Resident complained that on dates 2/9 & 2/11, she was taken to the shower and there was BM (bowel movement) in the shower and it smelled. Resident noted that staff did not clean shower after use. 2. Resident complained that when staff empties her roommate (sic) bed pan, they don't rinse the pan. They leave it in the bathroom and it makes the bathroom smells (sic). 3. Resident complained that staff does not empty trash can after completion of shift. -On 4/11/17 three residents filed a grievance as follows: Residents c/o (complain of) 1st shift leaving soiled towels and washcloths in the bathroom. Residents noted that staff leaves shift and sometimes they have to remove linen and trash bags from their rooms. Sometimes 2nd shift has to remove trash. (Resident's name) noted that sometimes 1st leaves her roommate (sic) briefs in the trash in the bathroom. -On 6/7/17 a grievance was filed by a family member alleging the resident's Bathroom had feces in (sic) the underside of the toilet. It took 3 days for HK (housekeeping) to clean this up. b. Review of the Resident Council Minutes for the past three months (May - (MONTH) (YEAR)) showed ongoing concerns with housekeeping services: -On 5/16/17 and 6/13/17 the Resident Council Meeting Minutes showed under Last Month's Minutes dirty towels & trash being left in room by 1st shift and issues with housekeeping under Outcome, Per res (resident), this is still an issue . Per res, no improvement. Manager went over cleaning procedure with res. 2020-09-01
