**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on record review and staff interview, it was determined that the facility failed to investigate the cause of a fracture for one resident (#3) from a total sample of 15 residents. Findings include: Resident #3 sustained a moderately displaced and mildly angulated spiral fracture to his/her left distal tibia and fibula that was identified by an x-ray report on 2/8/10. Licensed nursing staff documented in the 2/5/10 at 4:17 a.m. nursing notes that a certified nursing assistant (CNA) had observed that the resident's left ankle had [MEDICAL CONDITION] (swelling) and was painful with movement. However, review of the resident's record revealed no known etiology (cause) for the fracture. The licensed nurse coded the resident as requiring extensive assistance of one staff member (3/2) for bed mobility and dressing on the 12/8/09 quarterly Minimum Data Set (MDS) assessment. During an interview on 8/25/10 at 9:50 a.m., the Director of Nursing confirmed that the facility did not conduct a thorough investigation to determine the cause of the resident's fracture. |
