Findings include Resident #8 had a physician's orders [REDACTED]. However, a review of the resident's June 2010 Medication Administration Record [MEDICATION ADMINISTRATION RECORD DETAILS REDACTED]. However, review of the clinical record revealed that there was no evidence of a physician's orders [REDACTED]. During an interview on 8/31/10 at 3:30 p.m., licensed nurse "AA" confirmed that there was not a physician's orders [REDACTED]. However, nursing staff administered Ambien to the resident every night in June and July and 30 of 31 nights in August, 2010. Although the consultant pharmacist reviewed the resident's drug regimen in July and August 2010, she failed to identify the change in the the frequency of administration of Ambien without a physician's orders [REDACTED].
