In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation and record review, it was determined that the facility failed to ensure that one resident (#11) of 13 residents dependent on staff for transfers was appropriately transferred from the wheelchair to the bed from a total sample of 18 residents. Findings include: Resident #11 had a [DIAGNOSES REDACTED]. His/her care plan did not include any interventions to address his/her need for staff assistance to transfer. On 10/20/09 at 1:25 p.m., during an observation of the resident being transferred from his/her wheelchair to the bed, two certified nursing assistants (CNAs) inappropriately lifted the resident under his/her arms and by the waistband of the resident's pants. |
