In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This was evidenced by the failure to ensure safe storage of drugs and therapeutic agents, to prevent access by cognitively impaired residents for one of five (1 of 5) medication carts. Findings include: Observation on 9/22/09 at 3:35 p.m. revealed that the medication cart on the A Hall was unlocked. All drawers (except the narcotic box) were able to be opened. No staff was in attendance, or in the vicinity. The Nurse's Station was approximately 50-60 feet away. Licensed Practical Nurse (LPN) "GG" was observed approaching the cart from the Nurse's Station. In an interview with LPN "GG" on 9/22/09 at 3:40 p.m. she acknowledged that the medication cart should be locked when leaving it unattended. 2014-04-01
