In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include. Review of the discharge summary dated 6/01/10 for resident #164 revealed the resident was discharged from the hospital to the nursing home with a [DIAGNOSES REDACTED]. Review of Nurses Notes dated 6/23/10 revealed the resident was sent to the hospital for a cardiac defibrillator implant. He returned to the facility on [DATE] with the defibrillator to his left chest with steri-strips intact. There was no evidence in the record that a care plan was developed related to the care and monitoring of the defibrillator implant. Interview with the Licensed Practical Nurse (LPN) Minimal Data Set Assessment Coordinator on 9/22/10 at 8:25 a.m. confirmed there was no care plan for the resident's defibrillator and interventions should have been put into place when the resident returned with the defibrillator implant. 2014-04-01
